Critical neurologic complications spread through bony defect of lamina papyracea in sinusitis
Abstract
Introduction : Intracranial complications of pediatric sinusitis such as meningitis, epidural abscess, and cerebritis are rare but potentially life threatening. In such cases, inappropriate management may lead to severe brain damage or death. Case presentation : We experienced a case of subperiosteal, and epidural abscess of cranial fossa, and cerebritis which were spread through ipsilateral frontal and ethmoid sinusitis in an otherwise healthy immunocompetent boy. 5-year-old boy who was admitted with fever, generalized edema, and swelling on left periorbital area. Initial laboratory findings showed severe leukocytosis, elevated acute phase reactants. Also the findings of acute renal failure including hypoalbuminemia, azotemia and electrolyte imbalance were noticed. His mental status was clear and neurologic examination was normal without any focal neurologic sign. However, orbit and brain imagings showed pansinusitis, left orbital cellulitis, subperiosteal and epidural abscess spread though bony defect of lamina papyracea. The patient was treated with broad-spectrum antibiotics, but we delayed surgical treatment with the concern for deterioration of renal function by general anesthesia. Although he has been rapidly improved, fever was recurred at 10th hospital day. So we checked up brain magnetic resonance imaging in which new lesion of cerebritis was identified. Urgently, the patient underwent surgical treatment with functional endoscopic sinus surgery (FESS), at 12th hospital day and then he has recovered without neurologic sequela and his renal function was restored completely. Conclusion : We described a case of sinusitis which developed serious neurologic complications in a child with bony defect of lamina of papyracea.
